It is with mixed emotions that I announce closing of my practice, Medical Clinic at the Ranch effective 04/14/2022. I will not be available to see patients effective 04/15/2022. I made this decision to spend more time with my family and children. It has been a great pleasure and privilege to partner with you over the last few years to manage your health care needs and it is not easy for me to give up. I greatly value our relationship and thank you for your loyalty and friendship.

Please complete the attached Authorization form marked Medical Clinic at the Ranch only and mail or fax (281-371-0121) to my office as soon as possible so we may transfer your records to new physician.
EFFECTIVE 04/15/2022
We have retained CARIEND, a medical records custodian to store and manage records for the next 10 years. Effective 04/15/2022. please complete the attached Authorization form marked CARIEND only.
The fastest way to request copies of your medical records is to visit CARIEND's website and submit your contact information there. You will then be sent an email with instructions to securely upload your authorization form and ID.
